SOURCE FACT
OIG LEIE
The official Federal list says a named party is excluded, under a cited statutory authority, on a stated date.
EVIDENCE BEFORE ASSERTION
Your agency is exposed through the people you place and through the entities you place them into. Incumbent screening covers the first. ExcludeRadar covers both, and is explicit about which evidence class every output belongs to.

SOURCE FACT
The Federal provider registry says an NPI exists, when it was enumerated, and whether it is active.

HUMAN DECISION
A credentialing professional confirms identity and decides. ExcludeRadar never makes that call.
THE TWO LANES
The NPI lane. A National Provider Identifier is a unique national identifier. If the NPI on your roster appears in the OIG file, that is an exact match — no ranking, no judgement, no adjudication queue. This lane is fully automated and it is what the product is priced on.
The name lane. Most of a facility roster — aides, dietary, transport, temporary staff — has no NPI. Names are not identifiers, and matching them is fuzzy work with real consequences for real people. We rank candidates with the corroborating fields, show the match basis on every one, and hand them to your reviewer.
We will not sell you the name lane as automation. Any vendor who does is either hiding an offshore review team in their margin or handing you their false positives as your liability.
THE COUNTERPARTY CHECK
| Signal | An operating practice | A billing shell |
|---|---|---|
| Web operating history | Years of continuous observation | Days or weeks since first observation |
| Patient pathway | Booking, portal, EHR, or telehealth present | No way for a patient to reach it |
| Commercial infrastructure | Payments, business email, analytics | A template page and a TLS certificate |
| Footprint depth | Dozens of technologies | Two or three |
| NPI enumeration | Long-standing, active | Weeks old |
Any one of these alone means little — plenty of legitimate small practices have thin websites, and we score them as "insufficient evidence" rather than as suspicious. It is the combination, on a counterparty already sending you invoices, that is worth ten minutes of verification before Monday.
THE SAFETY BOUNDARY
These are product constraints, not disclaimers we hope you skip.
COVERAGE, STATED PLAINLY
Included today: the HHS OIG List of Excluded Individuals and Entities, refreshed from the official file; the CMS NPPES provider registry; and BuiltWith web-footprint observation for counterparty checks.
Not included today: SAM.gov exclusions, State Medicaid exclusion lists, licensure and board actions, and OFAC. State Medicaid lists are added per state as a priced add-on for the states you actually operate in — roughly forty of them exist, in inconsistent formats, and we would rather cover four of them properly than claim all of them.
If a competitor tells you they cover everything, ask them which State Medicaid lists are PDF-only and how often they re-parse them.
