WHAT YOU CANNOT SEE FROM PAPERWORK

The credentials look fine. They always do.

A shell operation supplies exactly the documents you ask for. It has an NPI, because NPIs are free and issued on application. It has an address, a tax ID, and a signature on your agreement. Nothing in that packet distinguishes it from a practice that has been treating patients since 2009.

What it does not have is history. It cannot manufacture ten years of observed technology on a domain, and it has no reason to buy a patient portal for patients it does not intend to see.

Web operating history is the strongest. BuiltWith has observed the public internet longitudinally for over two decades, so a domain's first-observed date is an operating record, not a WHOIS field that can be reset by re-registering.

Then the patient pathway: booking, patient portal, EHR, telehealth. An organisation that bills for care but gives patients no way to reach it is the core pattern. Then commercial infrastructure, footprint depth, and how recently the NPI was enumerated in the CMS registry.

Straight answers

Is a low score an accusation of fraud?

No, and the product will never say it is. A score is a web-footprint observation about a business. Plenty of legitimate small practices have thin websites, which is why sparse evidence is reported as "insufficient evidence" rather than as suspicion.

Do you scan the counterparty's systems?

Never. Every signal comes from passive, historical observation of the public web plus two Federal registries. Nothing touches the counterparty's infrastructure.

Can a shell fake these signals?

Buying a patient portal, running years of observable web history, and maintaining commercial infrastructure costs more than most shells are set up to spend, and history in particular cannot be backdated. It raises the cost of looking real, which is the point.

What if we think a score is wrong?

Every signal shows its underlying observation, so you can check our reasoning rather than trust it. Re-run the check after the footprint changes and the score moves with the evidence.
